Health: Think Like a Doctor: A Cough That Won’t Quit
The Challenge: Can you figure out what is wrong with a previously healthy 75-year-old woman with a cough that just won’t quit?
Every month the Diagnosis column of The New York Times Magazine asks Well readers to take on a difficult case and offer their solutions to a diagnostic riddle. This month’s case revolves around a lively, elderly woman — a patient of mine — who suddenly develops a cough that persists and worsens over the course of several months. I’ll provide you with the notes, labs and imaging that amassed over the half a year it took to come up with an answer.

The Patient’s Story:
“Dr. Sanders, I’m still sick.” The slender 75-year-old woman shook her head sadly. “I hate to be a bother, but you’ve got to help me with this cough. It’s just about killing me.”
Her face was pale; there were dark smudges beneath her eyes, and she’d lost a lot of weight. Whatever she had, if it wasn’t killing her, it certainly was making her sick.
The Patient’s History:
Her problems had started a few months earlier. She’d come in to see me with a simple cough. It had started with a cold, but that infection was long gone and all that remained was this cough.
It was strange, she told me back then. She’d be fine during the day, with the occasional cough — but at night, when she lay down, it was unbearable. Indeed, she had slept in a chair the past two nights because in bed she kept waking up to cough.
Then, the day before, she’d had this strange pressure in her chest during one particularly forceful paroxysm. That scared her enough to pick up the phone and make an appointment to see me. It must have been quite a fright, since I usually saw her no more than once a year. It wasn’t that she didn’t like me, she told me the few times I’d seen her in the past; she just didn’t like doctors.
Although she was worried at that initial visit, I wasn’t. Her report that she’d spent the past couple of nights sleeping in a chair might have been a concern in some of my patients, since one sign of heart disease is this inability to lie down flat, but this woman had no risk factors for heart disease other than her age. She didn’t smoke — and never had. And she was in great shape. At 75 she had no medical problems; she ate right, and she exercised almost every day. She was one of my healthiest patients.
Her physical exam was normal that first visit. So was her EKG. I figured she probably had some leftover bronchospasm from a resolved upper respiratory infection. Or maybe this was bronchitis. So I gave her an inhaler to calm her twitchy airways and a short course of antibiotics, and I really expected the cough to disappear.
But the cough persisted. Indeed, each time she returned to my office or talked by phone over the next few months, her cough was just a little bit worse and she was a little sicker. None of the tests I’d ordered were particularly revealing, and none of the medications I had prescribed had helped. She was starting to get frustrated, and so was I.
In the Exam Room:
Seeing her now, I was more than frustrated; I was worried. She’d lost a lot of weight. Her normally thin face looked gaunt, and the fat that normally pads the temples and forehead was gone. Her eyes were tired and puffy. And she couldn’t get more than a few words out before a new paroxysm of coughing cut her off.
She couldn’t sleep, she told me between bursts of her barking cough. And she had no appetite. Even when she was hungry, the constant coughing made eating almost impossible. She had a new symptom as well: abdominal pain.
On exam, once again, I found nothing. She had no fever. Her lungs were clear. There were no enlarged glands suggesting an unresolved infection. Her abdomen was soft and had no tenderness. However, now when she coughed, dark yellow mucus came up. Whatever she had, it was clearly getting worse.
“Can’t you give me something for this cough?” she asked me once more.
The Most Common Symptom:
Cough is the most common single reason for a visit to a primary care doctor. Most of the time it’s caused by an upper respiratory infection or cold. But that type of cough usually resolves on its own. A persistent cough — like my patient’s — was a different story. It had many possible causes. The most common was some kind of environmental allergy. I treated her with antihistamines. They didn’t help.
Heartburn (also known these days as reflux) can also cause a cough, as the acid from the stomach irritates delicate tissues never designed to withstand it. I tried a couple of different types of anti-acid medications. They didn’t help either.
She said she never wheezed, but she did have some chest tightness and on occasion felt short of breath. Was this asthma? I gave her another inhaler. It didn’t work any better than the first one.
Those three diagnoses — allergies, reflux and asthma — were why most people kept coughing, most of the time. She clearly didn’t have what most people had. So what was left?
Could it be lung cancer? She had never smoked, but 10 to 15 percent of those who are found to have lung cancer didn’t smoke. Could this be tuberculosis? She had traveled throughout rural Central America decades earlier, so she could have been exposed.
A chest X-ray might have shown either of those diseases — but didn’t. In fact, it was completely normal. So was a second one, done weeks later when she still was no better.
I sent her for pulmonary function tests. The report said her lungs weren’t exactly normal, but the technician commented that her constant coughing made the test difficult to interpret. You can read the pulmonary function test report below.
The Right Test:

Now she was back in my office, sicker than ever. I was frustrated and starting to get nervous. Her weight loss was worrisome. And she had this new abdominal pain.
I had already referred her to a gastroenterologist to look for any evidence of reflux or even a malignancy. She was to get scoped the following week.
In the meantime, I sent a sample of her purulent sputum to the lab to culture. When those results were also unrevealing, I decided to get a chest CT — and finally, finally found something abnormal.
Throughout the right lung there were areas of scarring and what is known as bronchiectasis, a particular type of scar tissue where the elastic component of the delicate airways appears stretched out and dilated, like a floppy waistband of a much-washed pair of shorts. (Bronchiectasis is visible in the area of magnification.)
Now I was worried and confused. Worried because this kind of lung injury made a patient prone to getting more infections. And confused because it wasn’t at all clear to me how this patient got it in the first place. Bronchiectasis is usually seen in patients who have a long history of serious lung infections. And this woman had had pneumonia a couple of times in the past decade, but otherwise nothing. I had an answer, a diagnosis, but not one that made sense. At least not right away.
It took me some time to figure it out, but I finally did.

For hundreds of years, coffee has been one of the two or three most popular beverages on earth. But it’s only recently that scientists are figuring out that the drink has notable health benefits. In one large-scale epidemiological study from last year, researchers primarily at the National Cancer Institute parsed health information from more than 400,000 volunteers, ages 50 to 71, who were free of major diseases at the study’s start in 1995. By 2008, more than 50,000 of the participants had died. But men who reported drinking two or three cups of coffee a day were 10 percent less likely to have died than those who didn’t drink coffee, while women drinking the same amount had 13 percent less risk of dying during the study. It’s not clear exactly what coffee had to do with their longevity, but the correlation is striking.
Other recent studies have linked moderate coffee drinking — the equivalent of three or four 5-ounce cups of coffee a day or a single venti-size Starbucks — with more specific advantages: a reduction in the risk of developing Type 2 diabetes, basal cell carcinoma (the most common skin cancer), prostate cancer, oral cancer and breast cancer recurrence.
Perhaps most consequential, animal experiments show that caffeine may reshape the biochemical environment inside our brains in ways that could stave off dementia. In a 2012 experiment at the University of Illinois at Urbana-Champaign, mice were briefly starved of oxygen, causing them to lose the ability to form memories. Half of the mice received a dose of caffeine that was the equivalent of several cups of coffee. After they were reoxygenated, the caffeinated mice regained their ability to form new memories 33 percent faster than the uncaffeinated. Close examination of the animals’ brain tissue showed that the caffeine disrupted the action of adenosine, a substance inside cells that usually provides energy, but can become destructive if it leaks out when the cells are injured or under stress. The escaped adenosine can jump-start a biochemical cascade leading to inflammation, which can disrupt the function of neurons, and potentially contribute to neurodegeneration or, in other words, dementia.
In a 2012 study of humans, researchers from the University of South Florida and the University of Miami tested the blood levels of caffeine in older adults with mild cognitive impairment, or the first glimmer of serious forgetfulness, a common precursor of Alzheimer’s disease, and then re-evaluated them two to four years later. Participants with little or no caffeine circulating in their bloodstreams were far more likely to have progressed to full-blown Alzheimer’s than those whose blood indicated they’d had about three cups’ worth of caffeine.
There’s still much to be learned about the effects of coffee. “We don’t know whether blocking the action of adenosine is sufficient” to prevent or lessen the effects of dementia, says Dr. Gregory G. Freund, a professor of pathology at the University of Illinois who led the 2012 study of mice. It is also unclear whether caffeine by itself provides the benefits associated with coffee drinking or if coffee contains other valuable ingredients. In a 2011 study by the same researchers at the University of South Florida, for instance, mice genetically bred to develop Alzheimer’s and then given caffeine alone did not fare as well on memory tests as those provided with actual coffee. Nor is there any evidence that mixing caffeine with large amounts of sugar, as in energy drinks, is healthful. But a cup or three of coffee “has been popular for a long, long time,” Dr. Freund says, “and there’s probably good reasons for that.”

Jean McFee Raichle, 94, is a remarkably cheerful woman. She lives in an assisted living center in Seattle staffed by aides who are warm and nurturing. She exists in the moment, mostly untroubled by her Alzheimer’s disease.
Only once in the past several years has her daughter, Marilyn Raichle, witnessed her become alarmed. That was when Marilyn was having problems at work and called to tell her mother about it.
“She could hear the stress in my voice, and I could feel her becoming anxious,” said Ms. Raichle, 63. “I saw that she was picking up on my emotions, and I just stopped right there.”
Ms. Raichle had stumbled onto a phenomenon experienced by many people with relatives who have Alzheimer’s: emotional contagion. The term refers to the way we sense the emotions of others through their facial expressions, tone of voice or body language, and reflect them without being conscious of doing so.
Babies have this innate ability to catch and mirror the emotions of those around them. And it’s not just humans who share this capacity: other primates, rodents and birds appear to have it, too, according to several scientific studies.
Now, new research published in Proceedings of the National Academy of Sciences finds that emotional contagion is heightened in people with mild cognitive impairment (M.C.I.) and Alzheimer’s. And the greater the level of cognitive deterioration, the more pronounced this emotional mirroring seems to be.
Several explanations may account for the finding, said Virginia Sturm, an assistant professor of neurology at the University of California, San Francisco, and lead author of the report.
As the hippocampus — the brain region that helps modulate emotional responses — atrophies in Alzheimer’s disease, emotional sensitivity may be heightened, she speculated. And the brain network associated with emotional connection and empathy may become more active as pathways involved in memory and internal reflection become impaired.
“These networks may have a reciprocal relationship” that’s not yet understood, Dr. Sturm said.
In their research, the San Francisco researchers asked caregivers, friends or relatives to fill out a questionnaire rating the emotional responsiveness of 62 people with M.C.I. and 64 with Alzheimer’s; 111 people with cognitive concerns served as a control group. Then, all the participants in the study received M.R.I. scans of their brains.
The scans showed that higher levels of emotional contagion were associated with atrophy in the hippocampus and other temporal lobe regions, predominantly on the right side. Those regions are involved in several social and emotional functions, including assessing the trustworthiness and perspectives of other people.
“Degeneration in these regions may make people less adept at identifying the emotions of others in pencil and paper tasks, but they’re still able to maintain emotional reactions and connections via other routes that don’t depend on higher-level cognition,” Dr. Sturm said.
Mary Sano, director of the Alzheimer’s Disease Research Center at Mount Sinai Medical Center in New York City, called the study “fascinating” and said, “We talk so much about the cognitive loss in M.C.I. and Alzheimer’s, but in fact the emotional disturbance is equally problematic.”
Irritability or emotional volatility in people with M.C.I. and Alzheimer’s are common; these may arise because “someone is picking up an emotional cue that they can’t put into a cognitive context and is therefore misunderstood,” Dr. Sano said.
Dr. Dylan Wint, director of education programs at the Cleveland Clinic’s Lou Ruvo Center for Brain Health in Las Vegas, said, “I often tell caregivers that as facts start to fade, feelings and tone of your interactions become more important.”
“A great deal of what our higher-order cognitive function does is inhibit or modulate our lower-level responses,” Dr. Wint continued. “As these higher-order functions fail, there’s less of a dampening effect and our instinctual responses rise to the surface.”
What can caregivers do? Dr. G. Allen Power, a geriatrician in Rochester, talks to groups around the country about this issue. “We have to be very aware of our body language — being centered, present, not being distracted — when we’re with someone with dementia,” he said. “If we’re not, they’ll pick up on it and it will begin to shut them down.”
“If we are calm, we can bring people with dementia to a calm place,” he said. “And if we are anxious, we can amp people up.”
Also, recognizing that people with dementia live in a world suffused with emotion can help caregivers understand the importance of identifying feelings that may underlie a patient’s behavior and respond to those, meeting needs that someone is no longer able to express directly.
If you’re getting frustrated with someone with dementia, try stopping and counting to 10, Dr. Power suggested. Think of a positive quality they have, and focus on that. “Any mind trick you can use to get a place of acceptance and being nonjudgmental makes a difference, because the person you’re caring for will sense that and respond to that,” he said.

Cardiac Arrest
A cardiac arrest can best be described as a sudden and abrupt loss of heart function. This can occur in a person regardless of whether they have been diagnosed with heart disease but if this occurs suddenly and unexpectedly, it is called sudden cardiac arrest. Unfortunately, many people are prone to sudden deaths that occur usually within minutes after the initial symptoms. There are many reasons that a person can die suddenly from cardiac arrest, but the most common is if the patient suffers from cardiovascular disease.
Sudden death caused by cardiac arrest usually occurs because the electrical impulses in the heart become extremely fast. This is known as ventricular tachycardia. If the electrical impulses becomes extremely chaotic, is called to ventricular fibrillation. An arrhythmia or irregular heart rhythm makes the heart to suddenly stop beating. Bradycardia is when a cardiac arrest occurs because the heart has slowed down the right. There are many factors, other than heart disease and heart attacks that are known to cause cardiac arrest. These include suffocation, electrocution, respiratory arrest and trauma.
In as little as 4 to 6 minutes after cardiac arrest begins both brain death and permanent death. But cardiac arrest are reversible if treated within minutes. This is usually done by giving the patient an electrical shock to the heart to restore a normal heartbeat. This process is called defibrillation. Unfortunately decreases for every minute that passes where a patient does not receive CPR and defibrillation, the chance of survival by 10%. You find that very few attempts to resuscitate a patient will succeed after a period of 10 minutes or more.
There are no official statistics showing the number of cardiac arrests that occur around the world each year, but it is estimated that over 95% of people who suffer cardiac arrest will die before they reach the hospital. If defibrillation can be given to victims within five to seven minutes, 45% chance of survival. You'll actually find that dramatically reduces the death rate of young people suffering from a cardiac arrest. But we have many reports in the press over the years where young athletes have died due to cardiac arrest. The most common cause of death from cardiac arrest in young people is hypertrophic cardiomyopathy. This is where the walls of the ventricle is far larger than they should be, and in most cases this type of heart disease is hereditary. Unfortunately, the pumping Chamber of the heart far less than it needs to be, and therefore the heart must work much harder to pump blood around the body.
Death by cardiac arrest can be prevented and is most commonly associated with those who suffer from heart diseases. Among the most vulnerable are women aged 40 years or over as either smoke, suffer from high blood pressure or diabetes. Another well-known risk is syncope, which is where an individual will suffer the loss of consciousness or fainting. There are many different reasons why someone might faint, will be most important that they have an abnormal heart rhythm. Then there is always the fear that the next episode can cause cardiac arrest.
You will find that the frequency of sudden cardiac arrest is very related to the actual frequency of coronary artery disease. To reduce the risk of sudden death, it is likely that many more public health initiative must be inserted. It makes a lot of sense that if the adult population is increasingly aware of hypertrophic cardiomyopathy, the appropriate screening and action can occur, thus reducing the risk of death.
Current statistics show that around 335,000 Americans die each year of cardiac arrest. Doctors have issued a "chain of survival" that should be followed to give the best chance for survival.

Casein hydrolysates
The majority of people who exercise regularly are more than aware of the importance of the post workout nutrition. By consuming post workout protein, you can build your muscles and improve your overall performance. Furthermore, this is a great way to maintain a positive nitrogen balance inside the body. The importance of whey protein in this process are well documented, and it is generally accepted that whey protein hydrolysate (WPH) are absorbed quickly by the body. WPH is therefore considered to be one of the most effective forms of protein that you can consume post-workout. The recent introduction of casein hydrolysate, however, seems to be cold stir among those in the know.
The hydrolysate can literally be translated "product of hydrolysis". Hydrolysis can be best described as a chemical process that causes one substance to be broken down into smaller units. A protein hydrolysates are therefore a mixture of amino acids that are prepared usually by breaking down a protein with acid, alkali or enzyme. The reason why an original protein is broken down, making it easily digestible and therefore easier for the body to absorb.
Grow muscle it has to be a positive nitrogen balance (protein). However, we are aware that the basic protein are not sufficient on their own. Scientific research has shown that a high plasma amino acid itself is inadequate to actually promote protein synthesis. It also tells us that we need a fair level of amino acids as metabolic process requires the most. This is, of course, post workout, and it's important to keep both levels of amino acids and high plasma insulin. In theory, a protein that the body is able to absorb more quickly to be the most effective form of amino acid after a workout.
Scientific study in 2009 showed that ingestion of a protein hydrolysate is much more likely to accelerate protein digestion and absorption in the gut, then a normal protein. In fact, one can increase the proportion of dietary amino acids Hydrolysate as part of skeletal muscle protein. During this study was the main form of the Hydrolysate of casein hydrolysates used. It was proven that this form of protein managed to increase levels of insulin, which in turn will increase the rate of protein synthesis. Furthermore, this is a fantastic way to put carbs in muscles. So it seems that casein hydrolysate is an easily digestible and absorbed protein, it is also able to increase the insulin and therefore will increase the rate of protein synthesis.
Many people strongly believe that the casein hydrolysate is far more effective than whey protein hydrolysates. The reason for this is that some glucogenic amino acids are, meaning that during the metabolic process, they will be converted into glucose. This will actually explain why certain amino acids are related to insulin response above. Whey protein is known to be lower in some amino acids such as arginine, glutamine and phenylalanine. These are three of the major glucogenic amino acids. Casein is known to have a high content of these amino acids and threonine, which would suggest that the casein hydrolysates to be much more effective than whey.
Unfortunately, there is no relevant studies to back up this theory. There are also some data that compare the digestion of casein hydrolysates and WPH. Therefore we can only assume that casein hydrolysates holds an advantage over WPH. Casein protein that has not changed is perfect to drink as a night shake. This is due to the fact that it is very slow is melted and absorbed in the body. The molecules of casein protein is known to sit in my stomach together over a long period of time, thereby slowing the rate of gastric emptying.
Casein hydrolysates derived mainly from casein protein's found in a cow's milk. You will find that a normal good-casein hydrolysates will contain about 85% protein. Casein enzymatic hydrolysis, and then usually undergo solution will be treated by heat. After this solution will be concentrated and spray dried. This all sounds scientifically possible, but will use casein hydrolysates make any difference to your post workout nutrition? The main advantage of casein hydrolysate is that it allows you to make the most of your post workout nutrition as it also contains carbohydrates. This will directly increase your insulin into the blood which, as mentioned, can make a big difference for you after your workout. Many people consider casein hydrolysates be best of all post workout protein. Many of us, however, is careful to spend large sums of money on supplements that may turn out to be worthless. With that said, there is plenty of evidence to prove that the casein hydrolysate is useful and therefore it may do no harm as part of your post workout nutrition.

DASH diet guidelines
The DASH diet, or Dietary Approaches to Stop Hypertension, was designed for those with hypertension or prehypertension; hypertension is the medical term for blood pressure. It is a low salt diet plan with additional benefits designed to reduce blood pressure. Not surprisingly, the concepts of this diet could be followed by anyone on a healthy eating plan because it includes a lot of fruits, vegetables and low-fat dairy. Eating Tips for the DASH Diet Plan
Make gradual changes
The slower you incorporate the changes, the more likely they will become habits and you will stick to them.
Limit sodium intake
Sodium intake should be between 1,500 and 2,300 mg per day, preferably somewhere near the lower end.
Limit consumption of certain fats
Trans fats, saturated fats, and cholesterol should be eaten on a limited basis. These fats are bad for the heart and can clog arteries.
Increase servings of produce and dairy
This diet includes plenty of fruits, vegetables and low-fat or nonfat dairy products.
Increase fiber and whole grain intakes
Fiber is great for the digestive system and has many other important functions; half of your grains should be whole grains.
Limit sugar intake
Refined sugar has become a big part of the American diet; try to reduce your consumption of added and refined sugars.
Reduce alcohol consumption. Alcohol is metabolized just like sugar.
Exercise
Exercise for at least 30 minutes every day. It doesn’t have to be 30 minutes all at one time; it can be broken into 10 minute intervals or something along those lines. The exercise should be moderate intensity, meaning you shouldn’t be able to complete a whole sentence while you’re exercising. Exercising regularly can aid in weight loss and help reduce blood pressure.
Beware of empty calories
Foods that don’t provide any nutritional value are designated as empty calorie foods. Empty calories, however, can also come from beverages, such as sodas. Drink water, herbal teas, healthy juices, and skim milk.

Don’t deprive yourself
If you’re craving a sweet, go for it. Just make sure it’s not an everyday thing. If you completely deprive yourself, you’ll end up bingeing at some point…and regret it!
Increase calcium, magnesium, and potassium intake
Foods that are rich in these, respectively include milk, cheese, green vegetables, fortified juices, and soy products; green vegetables, potatoes, milk, shrimp, nuts, and whole grains; spinach, potatoes, tomatoes, bananas, cantaloupe, pears, and fish.

Healthy hearts at Jennings Motor Group
When Colin Massey, sales manager, at Jennings Ford’s Cargo Fleet Lane super site in Middlesbrough, received a letter informing him about the Heart Check Screening available to staff, he thought it was an excellent opportunity for him and his colleagues to get the once over. Because the test is carried out on a block booking basis within the workplace, Colin had to find a minimum of 15 members of staff who fitted relevant criteria to take the test.
Members of staff had to be aged between 40 and 74, have no history of cardiovascular disease (including Stroke, Diabetes, and Heart Disease) and not had a Healthy Heart Check within the last five years. The number of staff at Jennings’ site in Midddlesbrough who volunteered for the test, exceeded the minimum total, but this wasn’t a problem for the NHS Trusts, who willingly accommodated the extra three members of staff who wanted to participate.
44-year-old, Colin, volunteered to be the first member of staff to undergo the test, which took only 20-30 minutes to complete and was carried out by a team of specialist nurses from the Primary Care Trusts. Each screening, which is entirely free to employers, consists of a simple finger-prick cholesterol test, blood pressure and pulse readings, in addition to a BMI (body mass index) height and weight calculation.
Following completion of the tests, Colin, was taken through a FINDRISC lifestyle questionnaire, which detects people at risk of type 2 diabetes, and at the end of the assessment, was given a cardio-vascular disease risk score. The results place the patient in one of three risk groups including low, medium or high. Gareth Harding, went on to say; “The patient is not diagnosed with cardiovascular disease on the day, they are simply given an indication of their personal risk of developing one of the vascular diseases including heart disease, stroke, kidney disease and type 2 diabetes within the next ten years.”
With consent, the patient’s GP is also informed of the results and if necessary, referrals to other NHS services are made. Throughout the screening, Jennings’ staff received general health advice on how to maintain or improve their health, which they found informative and beneficial. Staff were also given the opportunity to see a model of a clogged artery, which results from a build-up of a substance called plaque on the inner-walls of the arteries.
Colin Massey, sales manager at Jennings Ford’s Middlesbrough dealership said; “The Cardio Vascular Disease project is an excellent health initiative which has been set up by the Primary Care Trusts and is something my colleagues and I were more than willing to support with our participation.” Colin added; “A simple and painless 20 to 30-minute test, which identifies those at risk of developing cardiovascular illness, could effectively save someone’s life.” The successful screening at the Cargo Fleet dealership, resulted in a further 16 members of staff at the group’s Yarm Road site in Stockton, also taking part.
The Healthy Heart Check Workplace Screening Programme was launched in 2009 and started with only a small team of NHS nurses who carried out the checks. Today, the team consists of a nurse bank of over 30 specialist nurses performing assessments in workplaces spanning the entire Teesside area. In the last two years, the NHS Trusts has screened over 5,000 individuals spanning dozens of businesses and community venues across Teesside. Gareth Harding, concluded; “We have enjoyed successful screenings at a wide range of organisations, from local authority to chemical and process plants as well as large retail companies and schools and colleges. We have even helped groups of small businesses deliver the screening as part of a collaborative screening event.”
